About the Role
As an RN Case Manager, you'll work with clients, their families, and caregivers to ensure seamless, high-quality home care services. Your day-to-day responsibilities will include:
Responding to inquiries and conducting warm, in-person nursing assessments for new clients.
Creating personalized care plans and overseeing the smooth onboarding of clients.
Partnering with caregivers to ensure they are well-informed and confident in implementing care plans.
Maintaining regular communication with clients, families, and caregivers to monitor and adjust care plans as needed.
Building trusting relationships with clients and families while promoting the growth of the service within the local community.
What You Bring
To succeed in this role, you'll need:
A Bachelor's or Associate's Degree in Nursing from an accredited institution.
An active Registered Nurse license in Maryland.
At least two years of nursing experience (prior case management experience is a plus).
A growth mindset and a drive to build strong community relationships.
A commitment to service excellence and customer satisfaction.
Strong written and verbal communication skills, attention to detail, and organizational abilities.
The ability to travel within Baltimore County, Harford County, and Baltimore City to assess and visit clients.
