We are looking for a customer-service-oriented, compassionate Registered Nurse to join our team as an RN Case Manager. About the Role:
As an RN Case Manager, you'll play a key role in ensuring a positive experience for clients, their families, and other professionals involved in their care. You'll serve as the main point of contact for clients, from the initial inquiry all the way through the setup and ongoing management of care services.
Your Responsibilities:
Provide clients with a warm introduction to services by responding promptly to new inquiries and completing in-person nursing assessments.
Oversee the setup of services by creating care plans, matching caregivers to clients, and orienting caregivers to the client's care needs.
Ensure regular, proactive communication with all parties involved in the care process.
Revise care plans as needed to ensure they meet clients' evolving needs.
Build strong, trusting relationships with clients and their families, ensuring a positive experience.
Foster and maintain relationships with community referral sources, and promote the services in a creative, resourceful way.
What You'll Bring to the Role:
Bachelor's or Associate's Degree in Nursing from an accredited institution.
Registered Nurse (RN) license in the state of Maryland.
A minimum of two years of nursing experience.
Prior experience in case management is a plus, but not required.
Ability to travel within the Baltimore County, Harford County, and Baltimore City areas for client visits and assessments.
