Summary: St. MaryCorwin Hospital is looking for a locum to help fill 3 weeks FebMarch due to a locum needing some vacation time. Would be highly interested in a candidate who is seeking permanent placement if the right fit. Please disclose if the provider is interested in perm.

BID DUE DATE: Nov 29th at 5p EST!

Rate Caps for Submission:

• Malpractice Must be included in rates.
• Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
• Hourly BR and below Per hour not OT
• Holiday Must be 1.5x the regular rates Per holiday hour no OT

• Feb 1720 8a5p
• Feb 2427 8a5p
• Mar 36 8a5p
• Mar 10 8a5p

Details:

• MondayThursday 8a5p with a 1 hour unpaid lunch.
• Setting: Clinic Trauma Level III
  
• FTE: 0.8
• Required Procedures/Job Duties: Some rare biopsies and bone morrow
• The locum will also provide oversight to their infusion clinic who sees 3545 patients per day.
  
• EMR: Epic
• Support Staff: RN MA
• Reason for Coverage: Current locum requested vacation time.

EMR Required Training:

• Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
• EMR Virtual training Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day.

Preferred Lodging: Preferred hotels

Air Travel: Local no flights

Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

Requirement description :

• Board Certified Medical Oncology Required or Truly BE (05 years of residency completion)
• Active CO License Highly Preferred
• Active BLS Required
• Active DEA Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
• Candidate must be available for all dates detailed in job description to be considered Required
• Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date Required
• Clean Malpractice/background Highly Preferred must provide Malpractice/background explanation
