About the Role:
This full-time, work-from-home position offers a unique blend of autonomy and community connection. You'll travel within the Central, VA region to conduct face-to-face home visits, providing vital support and care to our members.
As an RN Clinician specializing in OBGYN, you will provide comprehensive case management services, developing, monitoring, evaluating, and revising care plans tailored to the unique needs of each individual. Your focus will be on optimizing health outcomes throughout the entire pregnancy journey and beyond. You will:
Develop and implement personalized care plans that address the physical, behavioral, social, and long-term care needs of pregnant and postpartum women.
Perform telephonic and in-person clinical assessments to identify, evaluate, and manage member needs.
Coordinate care for high-risk members, including those with chronic conditions, co-morbidities, or disabilities.
Manage care to ensure cost-effective and efficient utilization of health benefits, including conducting gap-in-care management for quality programs.
Facilitate authorizations, referrals, and care plan implementation.
Collaborate closely with Medical Directors, Physician Advisors, and Interdisciplinary Teams to discuss case management treatment plans.
Ensure compliance with all relevant regulations, accreditation standards, and company policies.
Assist with problem-solving related to provider, claims, or service issues.
Provide specialized care and support to pregnant and postpartum women, with a focus on maternity care, including labor and delivery, postpartum care, and mother/baby care.

What We're Looking For:
Active Registered Nurse (RN) license, with a Compact/Multi-State License.
Bachelor of Science in Nursing (BSN) preferred.
Nursing experience, with a focus on case management, care coordination, or patient advocacy, specifically with medical/clinical cases for pregnant and postpartum women.
Maternity experience (Labor & Delivery, Postpartum, Mother/Baby) is highly preferred.
Strong understanding of the physical, psychological, socio-cultural, and cognitive needs of pregnant and postpartum women.
Excellent communication, problem-solving, and analytical skills.
Ability to work independently and collaboratively in a remote environment.
