The ideal candidate will provide high-quality physical therapy services to patients in the comfort of their homes. The Home Health Physical Therapist will assess patients' needs, develop personalized treatment plans, and implement interventions to improve mobility, alleviate pain, and enhance overall quality of life. This role requires excellent communication skills, strong clinical judgment, and the ability to work independently within a multidisciplinary team. Responsibilities: * Conduct comprehensive assessments of patients' physical abilities, including range of motion, strength, balance, and gait. * Develop individualized treatment plans based on assessment findings, patient goals, and physician recommendations. * Administer therapeutic exercises, manual therapy techniques, and modalities to promote functional independence and improve mobility. * Educate patients and their caregivers on proper techniques for performing exercises and activities of daily living. * Monitor patients' progress and modify treatment plans as needed to achieve therapeutic goals. * Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure continuity of services. * Document evaluation findings, treatment interventions, and patient progress accurately and timely in electronic medical records. * Maintain compliance with state regulations, Medicare guidelines, and agency policies and procedures. * Participate in ongoing professional development activities to stay current with evidence-based practices and advancements in the field of physical therapy. Qualifications: * Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program. * Current GA state licensure as a Physical Therapist. * Minimum of one year of experience working as a Physical Therapist, preferably in a home health or rehabilitation setting. * CPR certification is required. * Strong clinical assessment and critical thinking skills. * Excellent interpersonal and communication skills. * Ability to work independently and manage time effectively. * Proficiency in using electronic medical records systems. * Valid driver's license and reliable transportation.
