ADRC Community Health Worker

Hourly Range: $23.03 - $31.61

The Aging and Disability Resource Center (ADRC) of Milwaukee County Department of Health & Human Services provides FREE information and assistance regarding community resources and services for adults. ADRC staff assist individuals to plan for short-term and long-term needs, access community services and resources, and learn about programs and benefits available. Promoting Independence and Inclusion.

Job Summary: The Community Health Workers are the bridge that connect customers to care services and resources to support their health journey. Community Health Workers are community members who have lived experience in overcoming barriers to access, navigating systems, and utilizing resources in the communities they serve. The goal of this position is to improve health outcomes of people in their communities by addressing the well-being of the whole person.

Job Duties:

  • Collaborate with Progressive Community Health Centers to provide customer and system navigation, including scheduling follow up visits, issue reminders, self-management support, deliver patient education, etc.
  • Provide chronic disease and preventative guideline education to customers. Facilitate educational group session as needed. Work with customers to gain insights regarding barriers to care services or screenings.
  • Maintain clear, thorough, accurate, and objective documentation of contacts with customers.
  • Complete 100%-time reporting and encounter data reporting.
  • Perform other duties as assigned.
Minimum Qualifications:

  • Current resident of the State of Wisconsin.
  • High School Diploma or GED equivalent required.
  • At least one (1) year of experience working in the community in a community health worker position or similar role.
  • Candidates must understand basic public health care, including health promotion and maintenance, chronic disease prevention, and self-care management at the community level.
  • Must have in depth understanding of HIPPA and patient confidentiality.
  • Must have a Community Health Worker Certification from Milwaukee AHEC or through an equivalent state program.
Preferred Qualifications:

  • Associate degree.
  • Bilingual in Spanish or Hmong.
Knowledge, Skills, and Abilities: Skills needed for success in this role include customer focus, excellent verbal communication skills, patience, ability to write succinctly, professionalism, critical thinking, strong organizational skills, and time management skills. The ability to work collaboratively with other units and organizations within and throughout the county. Excellent credibility in the community. Ability to relate well to people of various ages from diverse ethnic and cultural backgrounds.
