Job Description:
Job Title: Care Manager (RN)
Duration: 3 months (Contract To Hire)
Location: Remote - Florida
Shift: 8am-5pm (Mon-Fri)

Telephonic Care Management outreaching to caregivers for pediatric experience (gathering updates from parents, caregivers) Understanding what care coordination is needed understanding the total picture so that AHCA can be updated appropriately per monthly basis.

Minimum caseload would be 30 ~ based on volumes

Dealing with Medically complex pediatric members can be very time consuming and requires the ability to multitask.
The CM candidate will need to be organized, self-motivated, possess excellent communication and time management skills, and possess critical thinking and problem-solving skills
Note:
As per the Plans AHCA contract the Candidate MUST have a minimum of 2 years' clinical pediatric experience
